Those types of places certainly aren't nearly as prevalent as they used to be, but there are more out there than you'd imagine. The ones in pharmacies are rare; but not fully extinct yet. But there are a lot of fifties theme restaurants with soda fountains and all kinds of shakes and malts out there.
As far as ice cream goes, I need to get to Milwaukee at some point. They seem to be the genuine frozen custard (as opposed to soft ice cream that's called frozen custard) capital of the country.
